NOTE:  A 10% late fee  will apply to returning players after June 1st.  Late registrants, whether returning or new, will be accepted only if space is available in the appropriate level.

New Registrants must register with Cassandra. 

Payment for online registration is as follows:
1.    PAYABLE:  Make cheques payable to SDMHA.
2.    REFERENCING:  Include your child's name in the Notes portion of your cheque.
3.    FUNDRAISING PAYMENT:  Prepare one cheque for $175 (fundraising portion) dated same day as online registration.
4.    REGISTRATION PAYMENT: Prepare a second cheque (or cheques) for the registration portion (see fees below).  These cheque(s) must be dated for the 1st of the month, with the final cheque dated no later than September 1st.
5.    PAYMENT in PERSON:  Cheques can be delivered in person to either the Registrar, Cassandra Barry, (613) 543-3465, or the Treasurer, Pam Mullin (613) 652-4498 or dropped off at KBD Transportation in Iroquois. 
6.    PAYMENT by MAIL:  Cheques can be mailed to SDMHA, PO Box 494, Morrisburg, Ontario.  K0C 1X0.
